How to Prefill Fields on Fillable PDF's

Prefilling Fields Based on Field Names


Certain fields on a fillable PDF can be prefilled automatically through the Portal. This can be set up by editing the names of the fields you would like to prefill through Adobe so that they match the names below. Here is a list of fields that can be automatically populated: 

  • FirstName: This will populate the Employee's First Name. 
  • MidName: This will populate the Employee's Middle Name. 
  • MiddleInit: This will populate the Employee's Middle Initial. 
  • LastName: This will populate the Employee's Last Name. 
  • SSN: This will populate the Employee's Social Security Number. 
  • Address1: This will populate the Employee's Main Street Address. 
  • Address2: This will populate the Employee's Secondary Address.
  • City: This will populate the Employee's City. 
  • State: This will populate the Employee's State. 
  • Zip: This will populate the Employee's Zip Code. 
  • Datepicker: This will allow the employee to select a date from a calendar, rather than manually entering it. 
  • EmployerName: This will populate the Company Name. 
  • EmployerAddress: This will populate the Company Street Address.
  • EmployerCity: This will populate the Company City. 
  • EmployerState: This will populate the Company State.
  • EmployerZip: This will populate the Company Zip Code. 
  • FEIN: This will populate the Employer's FEIN.
